Job Description
Job Title: Case Manager
Program: Fillmore Encampment Outreach & Case Management
Reports To: Shelter and Housing Program Manager
Status: Regular—Full Time
Type: Non-Exempt Hours/Week: 40 FTE: 1
Pay: $25.00 per hour + Bilingual Bonus
Position Summary
Case Manager will use culturally competent approaches to perform outreach to engage members of the Fillmore homeless encampments. Engagement efforts will include building rapport and familiarizing clients with case management process. Utilizing harm reduction model as well as trauma informed care practices, Case Manager will complete intake, develop individualized service plan, provide counseling and case management services. This includes referring to community resources. Case Manager will provide or work collaboratively with other TPF team members to provide housing navigation, health navigation, financial navigation, Homeless to Home Rapid Re-Housing, and other services as necessary.
Responsibilities
Outreach
a. Establish professional relationships with members of the Fillmore homeless encampments with the goal of offering services that will set them up for success in their own goals and transition into appropriate housing situations.
b. Identify potential program participants and provide them with information regarding TPF services and other services.
c. Perform intake and assessments with new clients.
Case Management
a. Develop an individualized service plan with client. This service plan should be based on the client’s expressed wants and needs.
b. Assist clients in obtaining mental health services.
c. Assist clients in obtaining drug and alcohol services as appropriate.
d. Assist clients in acquiring appropriate benefits, including SSI, SDI, GR, Veteran’s Benefits, or others as appropriate.
e. Assist clients in acquiring needed medical and dental care.
f. Coordinate with county mental health case managers, and other service providers. Assist clients in communicating effectively with their mental health team.
g. Assist clients in obtaining appropriate housing.
h. Assist clients in obtaining employment as appropriate.
i. Provide transportation to medical appointments, social security office, and county agencies, etc. as necessary based on clients’ inability to use public transportation.
j. Facilitate acquisition of other agency services required by client.
Life Skills
a. Educate clients about physical health and preventative care.
b. Educate clients about mental health, including coping skills, potential symptoms, treatment options, and helpful resources.
c. Assist clients in developing other life skills such as effective interpersonal skills, sobriety skills, symptom management skills, etc.
Mental Health Services
a. Maintain awareness of clients’ moods, behaviors, and actions--noting changes in client chart.
b. Evaluate and manage crisis situations in a calm and timely fashion; make determination of when the crisis team or police intervention is appropriate and proceed accordingly after consulting with case manager or program manager if safe and possible.
c. Educate clients about medication management if applicable, specifically side effects, benefits, compliance, and how to communicate medication problems effectively to physician.
Miscellaneous
a. Maintain client files.
b. Stay abreast of local resources.
c. Attend staff meetings and other meetings as assigned.
d. Additional duties as assigned by program manager.
Qualifications
a. Education in the areas of mental health, psychology, counseling, or addiction studies or equivalent experience preferred.
b. Three years or more of experience providing supportive services to severely and persistently mentally ill adults, preferably within a psychosocial rehabilitation model, preferred.
c. Peer lived experience desired.
d. Demonstrated effectiveness in addressing complex consumer issues.
e. Excellent written and verbal communication skills and ability to communicate with staff, clients, and other agencies in a professional manner.
f. Bilingual/Bicultural skills required.

a. Valid California Driver’s License
