OverviewCatholic Health is seeking an experienced Lead Performance Improvement, Education & OASIS Coordinator (RN) to support agency-wide quality improvement, clinical education, regulatory compliance, and OASIS integrity within our home care division.

Responsibilities
- Lead performance improvement and regulatory compliance initiatives
- Oversee OASIS documentation accuracy and ICD-10 coding compliance
- Analyze quality metrics including OBQI/OBQM, HHCAHPS, infection surveillance, and clinical documentation trends
- Educate and support clinical staff through orientation, training, and ongoing education
- Collaborate with leadership to improve patient outcomes and operational performance
- Support Joint Commission, CMS, and New York State compliance readiness
Qualifications
- Current RN license and registration required
- Bachelor’s degree required; Master’s preferred
- Minimum 5 years of CHHA/Home Care experience
- OASIS Certification required or in progress
- Proficiency in Allscripts and Microsoft Office applications
Posted Salary RangeUSD $110,000.00 - USD $125,000.00 /Yr.
