Direct Report: Director of Nursing/ Licensed Practical Nurse
Job Summary:
Under the supervision of the Director of Nursing and Licensed Practical Nurse you are responsible for providing the highest quality of care in our 117-bed facility to our residents in a family-like environment. You will record all patient interactions through thorough documentation and assess their well-being while following policies and procedures set forth by the facility and federal guidelines.
Qualifications:
- Successful completion of an accredited Certified Nursing Assistant program.
- Currently licensed as a CNA in the Commonwealth of Pennsylvania.
- Prior knowledge or experience with Point Click Care preferred but not required.
- Prior experience in long-term care preferred but not required.
- Ability to walk or stand for long periods of time.
Duties and Responsibilities:
- Monitor and record residents’ vitals and well-being, reporting any needs to the LPN or RN.
- Maintain the cleanliness and comfort of each resident, by assisting residents with bathing, feeding, dressing, and moving from wheelchairs, beds, and gurneys.
- Documenting resident records in a timely and confidential manner by signing and dating all entries.
- Complete and chart all accident/incident reports involving residents. Report all incidents involving staff to LPN, RN Supervisor or Director of Nursing.
- Receive/give report at the beginning and end of shift.
- Coordinate with ancillary departments to provide services and resources.
- Review care plans daily to ensure that appropriate care is being rendered by all staff.
- Ensure that rooms are ready for new admissions.
- Assist with restorative efforts set forth by the care team.
- Assist/go on transport with residents.
- Inform residents and families of treatment plans and procedures.
Specific Requirements:
- Must possess kindness, compassion, and patience for our residents and their families.
- Knowledge of nursing best practices in compliance with state, federal, and local standards.
- Must be able to work as a team.
- Must be able to read and write the English language.
- Must possess excellent oral and strong written communication skills.
- Must possess excellent time management, organizational, and multitasking skills to complete daily tasks.
Physical Requirements:
- Must be able to sit, stand, bend, lift and move intermittently throughout the workday.
- Must be able to lift a minimum of 50 pounds.
- Must be able to work independently.
