SUMR Labs - Full-Stack CTO
Company Overview
SUMR Labs is a Stanford-founded, clinician-built AI company developing clinical documentation intelligence (CDI) for hospital medicine. The platform combines rule-based logic with AI and NLP to identify clinically qualified conditions within EHR data, surface supporting evidence, and generate medically justified documentation drafts for physician review.
Note: SUMR Labs is a remote-first company. We collaborate primarily online but meet in person periodically for strategy sessions and product workshops. Bay Area–based candidates are preferred for ease of in-person collaboration, though not required.
Having completed its MVP, SUMR Labs is entering the validation and pilot-deployment phase with real clinical data. In Q1 2026, the company will begin a data-validation collaboration with a leading U.S. health system to test and refine its algorithms, alongside a live hospital-medicine pilot with a separate health system to evaluate workflow impact and ROI.
This is a hands-on build stage. The incoming CTO will work directly with production environments and live hospital datasets, leading integrations, refining agent logic in response to clinical feedback, and preparing SUMR’s platform for scale and commercialization.
Role Overview
The Chief Technology Officer (CTO) will lead SUMR’s end-to-end technical architecture, development, and integration strategy. This founding-level role is responsible for unifying SUMR’s agent logic framework and summarization pipeline into a scalable, compliant platform deployable across health-system environments.
Core Responsibilities
1. Python Architecture & Agent Logic
- Design and implement SUMR’s clinical reasoning engine, a modular Python framework that codifies diagnostic qualification rules, condition hierarchies, and clinical-evidence scoring.
- Build reusable “agent” modules that reason over structured (FHIR/OMOP) and unstructured EHR data, using deterministic logic augmented by AI outputs.
- Ensure auditability, versioning, and explainability of each logic layer for compliance and clinical validation.
- Develop and maintain data-mapping pipelines and schema alignment (SQLAlchemy / Pandas / PySpark) with familiarity in EHR data dictionaries and ontology normalization (ICD, SNOMED, LOINC).
2. AI / NLP Summarization Layer
- Integrate rule-based and model-based summarization for physician documentation support.
- Build connectors to LLM APIs or deploy custom fine-tuned models (OpenAI, Anthropic, Hugging Face) with prompt-chaining and retrieval logic.
- Oversee hybrid inference workflows where Python agents pre-process inputs and LLMs generate clinical narratives.
3. Full-Stack & Infrastructure
- Lead full-stack development using Python (FastAPI) backend and React/Next.js frontend.
- Architect secure data pipelines, APIs, and EHR integrations (FHIR, HL7, REST).
- Own HIPAA/SOC 2-compliant cloud infrastructure (AWS preferred), including access control, logging, and PHI encryption.
4. Technical Leadership
- Establish engineering best practices (GitHub Actions CI/CD, automated testing, documentation).
- Build and mentor a small hybrid engineering team (onshore/offshore).
- Collaborate closely with clinicians and product leadership to translate workflow insights into technical priorities.
Required Experience
- 8+ years in software engineering, with deep hands-on Python expertise (data pipelines, rule engines, or agent frameworks).
- Proven track record leading full-stack SaaS or data-intensive builds from architecture through deployment.
- Experience with EHR data standards (FHIR, HL7, OMOP) and healthcare compliance frameworks (HIPAA, SOC 2).
- Comfortable with schema mapping and data dictionary management for EHR integrations.
- Proficiency in integrating LLM-based or NLP pipelines with deterministic rule logic.
- Prior startup or founder-level experience preferred; must be capable of coding, designing, and shipping independently.
Key Attributes
- Architect-Builder: designs robust Python systems that encode clinical reasoning.
- Cross-Disciplinary: bridges medical logic, AI, and enterprise-grade engineering.
- Pragmatic: builds iteratively with reliability, compliance, and explainability in mind.
- Mission-Driven: passionate about improving clinical workflows and data integrity.
Compensation & Structure
- Founding-level equity with milestone-based vesting.
- Our fundraising round is anticipated to close by Q2 2026. A small monthly stipend will be available pre-funding. Salary ($60K-$100K range) will begin upon funding, transitioning to a competitive full-time paid role post-financing. Expected to scale to a full-market salary ($120K-$150K) as the company grows post-financing.
- Reports to CEO; collaborates closely with COO and clinical advisors.
- Focus through 2026: deliver pilot-ready platform with validated agent logic, NLP integration, and scalable, secure infrastructure.
